Getting started

Four steps from first call to first day

  1. Reach out

    A family member, the person, or a support coordinator calls or sends the form below.

  2. Meet and listen

    We meet to learn about the person — what they enjoy, what they want, and what they need.

  3. Plan together

    We build a support plan with the person and their team, based on their own goals.

  4. Begin support

    Services start with a consistent team. We check in and adjust as goals change.
